The A.M. McGregor Home: McGregor is a benefactor and charitable provider of housing and related services that meet the highest standards for quality, efficiency, innovation and compassion for seniors in Cleveland and northeastern Ohio.

The Atlantic Philanthropies (USA), Inc.: The Atlantic Philanthropies work globally to identify and support leaders, institutions and organizations dedicated to learning, knowledge building and solving pressing social problems. It has a lon-standing interest in aging.

The California HealthCare Foundation: The California HealthCare Foundation is an independent philanthropy committed to improving the way health care is delivered and financed in California. By promoting innovations in care and broader access to information, its goal is to ensure that all Californians can get the care they need, when they need it, at a price they can afford. CHCF commissions research and analysis, publishes and disseminates information, convenes meetings of key health care groups and funds development of programs and models aimed at improving health care in California.

The Charles Stewart Mott Foundation: The Charles Stewart Mott Foundation is a charitable organization, which administers funds through four programs - Civil Society, Environment, Flint Area and Pathways Out of Poverty. It also funds exploratory and special projects and is committed to supporting projects that promote a just, equitable and sustainable society. It supports nonprofit programs throughout the United States and, on a limited basis, internationally.

The Cleveland Foundation: The Cleveland Foundation is a community foundation whose mission is to enhance the lives of all residents of greater Cleveland, now and for generations to come, by building community endowment, addressing needs through grant making and providing leadership on key community issues.

The Commonwealth Fund: The Commonwealth Fund is a private foundation that aims to promote a high-performing health care system that achieves better access, improved quality and greater efficiency-particularly for society's most vulnerable individuals, including low-income people, the uninsured, minority Americans, young children and elderly adults.

Commonwealth Corporation of Massachusetts: Commonwealth Corporation is a quasi-public organization responsible for administering and delivering a wide range of public and privately funded initiatives designed to meet the labor needs of businesses; improve current and emerging workers' skills; foster career success through lifelong learning; and retain, sustain, improve and create job-generating businesses.

Daughters of Charity Foundation: The Daughters of Charity Healthcare Foundation of St. Louis supports programs that impact people who are poor and vulnerable in order to improve behaviors, enhance healthy lifestyles and enable individuals and communities to help themselves. The foundation accomplishes this in the St. Louis metropolitan area through programs that primarily focus on dental services, seniors living independently and child abuse and neglect prevention.

Duke Endowment: The Duke Endowment is a private foundation serving the people of North Carolina and South Carolina by supporting selected programs of higher education, health care, children's welfare and spiritual life.

Invacare Corporation: Invacare Corporation is the world's leading manufacturer and distributor in the $8.0 billion worldwide market for medical equipment used in the home. The company designs, manufactures, and distributes an extensive line of health care products for the non-acute care environment, including the home health care, retail, and extended care markets. Invacare sells its products to over 25,000 home health care and medical equipment providers, distributors, and government locations. The company has 5,700 associates and markets its products in 80 countries around the world.

Joan Anne McHugh Memorial Foundation: In February 2005, the American Association of Homes and Services for the Aging received funding from the Joan Anne McHugh Memorial Foundation to establish a nursing academy to honor the late Joan McHugh and her commitment to the long-term care field. Although the Foundation no longer exist, we continue to receive support from Joan Anne McHugh’s family, friends and former colleagues.

The John A. Hartford Foundation: The John A. Hartford Foundation is a committed champion of health care training, research and service system innovations that will ensure the well-being and vitality of older adults. Its overall goal is to increase the nation's capacity to provide effective, affordable care to its rapidly increasing older population. Today, the foundation is America's leading philanthropy with a sustained interest in aging and health.

The Langeloth Foundation: The Langeloth Foundation's is and organization which purpose is to promote and support effective and creative programs, practices and policies related to healing from illness, accident, physical, social or emotional trauma and to extend the availability of programs that promote healing to underserved populations.

The Foundation is particularly interested in funding programs that address the health of individuals who, because of barriers to accessing care, experience poor and sub-optimal health, including: those with no or severely limited income, cultural differences, lack of English language skills, lack of health insurance or inadequate health insurance, limited access to health care services, mental illness, substance abuse, homelessness, incarceration, and exposure to trauma.

Lutheran Services in America: Lutheran Services in America (LSA) is an alliance of the Evangelical Lutheran Church in America, The Lutheran Church - Missouri Synod, and their related social ministry organizations. LSA strengthens and adds value to the ministries of its member organizations, facilitates ministries best done together rather than as individual organizations, and enhances the witness of Lutheran social ministry. The national office is located at the Lutheran Center in Baltimore, Maryland.

LSA's more than 300 health and human service organizations provide care in thousands of communities in the United States and the Caribbean. Last year, these organizations served more than 6 million unduplicated clients, meaning that they served one in 50 people in the service territory. Utilizing the skill and dedication of a quarter of a million staff and volunteers, LSA member organizations provide services ranging from health care to disaster response, from services for children and families to care for the elderly, from adoption to advocacy.

McGregor Foundation: The McGregor Foundation, established in 2002 as a private grantmaking foundation, evolved from a history of more than 125 years of providing distinguished care for seniors at the A. M. McGregor Home in East Cleveland and the Amasa Stone House in the Glenville neighborhood of Cleveland. Tootie McGregor Terry founded The A.M. McGregor Home (McGregor) in 1904 in honor of her late husband, Ambrose, as a rest home for "gentlewomen who could no longer care for themselves." Ambrose McGregor, a close associate of John D. Rockefeller, had served as president of Standard Oil of Ohio. In 1999, the McGregor Board of Directors adopted a new strategic direction for the organization to specifically focus on "supporting seniors in need and those who serve them." The McGregor Foundation, with a separate Board and endowment, carries McGregor's mission to all of Cuyahoga County beyond the organization's East Cleveland campus. The McGregor Foundation and The A.M. McGregor Home comprise The A.M. McGregor Group.

The Milbank Memorial Fund: The Milbank Memorial Fund is an endowed operating foundation that works to improve health by helping decision makers in the public and private sectors acquire and use the best available evidence to inform policy for health care and population health.

Murray and Sydell Rosenberg Foundation (Greystone): The Murray and Sydell Rosenberg Foundation is a private foundation that focuses on alleviating the pain and suffering of severely impoverished individuals in many different parts of the world. It currently helps families with the basic necessities of life in the United States, former Soviet Union, South America, Central America, Israel and Africa.

The National Commission for Quality Long-Term Care: The National Commission for Quality Long-Term Care is a non-partisan independent body charged with improving long-term care in America. The appointed commissioners reflect a diversity of experience in government, academia, quality improvement and long-term care. Convened in October 2004, it grew out of an industry-led quality initiative called Quality First, A Covenant for Healthy, Affordable and Ethical Long-Term Care. Funding for the commission's work is provided by the Alliance for Quality Nursing Home Care, the American Health Care Association and the American Association of National Quality Forum; however, it is now an independent commission at the New School.

Retirement Research Foundation:The Retirement Research Foundation is the nation's largest private foundation devoted solely to serving the needs of older persons in the United States and enhancing their quality of life. For more than 25 years, RRF has been at the forefront of efforts to meet the ever-changing needs of older persons. It has invested more than $160 million to help build a network of innovative and skilled individuals and institutions committed to addressing aging and retirement issues.

The Robert Wood Johnson Foundation: As the nation's largest philanthropy devoted exclusively to improving the health and health care of all Americans, the Robert Wood Johnson Foundation works with a diverse group of organizations and individuals to identify solutions and achieve comprehensive, meaningful and timely change. The foundation supports training, education, research (excluding biomedical research) and projects that demonstrate the effective delivery of health care service.

The Sunflower Foundation: The Sunflower Foundation is dedicated to improving the health and well-being of all Kansans through improved access to affordable and quality health care, promotion of healthy lifestyles and prevention efforts. The foundation supports programs that are innovative and have the potential to affect change at a systems level. The Sunflower Foundation funds grantees that will have a positive impact on the health of Kansans.

Thrivent Financial for Lutherans Foundation: The Thrivent Financial for Lutherans Foundation is a private foundation formed in 1982. It is funded exclusively by contributions from Thrivent Financial for Lutherans and its subsidiaries.

The Foundation operates exclusively for charitable, educational and religious purposes to help improve the health and vitality of its key communities--the Lutheran community and the Fox Cities in Wisconsin and the Twin Cities in Minnesota.

U.S. Department of Health and Human Services:

• Agency for Healthcare Research and Quality: AHRQ is one of 12 agencies in the U.S. Department of Health and Human Services that responds to the challenges faced by our health care system. The agency's mission is to improve the quality, safety, efficiency and effectiveness of health care for all Americans and support efforts to translate research into practice.
• Health Resources and Services Administration: HRSA is the primary federal agency for improving access to health care services for people who are uninsured, isolated or medically vulnerable. It provides leadership and financial support to health care providers in every state and U.S. territory. Administration grantees provide health care to uninsured people, people living with HIV/AIDS, pregnant women, mothers and children. It also trains health professionals and improves systems of care in rural communities.
• Centers for Medicare and Medicaid Services: CMS administers the Medicare program and works in partnership with state governments to administer Medicaid, the State Children's Health Insurance Program and health insurance portability standards. In addition, CMS oversees administrative simplification standards from the Health Insurance Portability and Accountability Act of 1996, quality standards in long-term care facilities through the survey and certification process and clinical laboratory quality standards under the Clinical Laboratory Improvement Amendments.
• Centers for Disease Control and Prevention: CDC's mission is to promote health and quality of life by preventing and controlling, disease, injury and disability. CDC is committed to achieving true improvements in people's lives by accelerating health impact and reducing health disparities. It has created a set of four overarching Health Protection Goals, which are supported by a number of strategic goals and objectives.
• Office of the Assistant Secretary for Planning and Evaluation: ASPE provides policy analysis and advice; guides the formulation of legislation; coordinates strategic and implementation planning; conducts regulatory analysis and reviews regulations; oversees the planning of evaluation, non-biomedical research and major statistical activities; and administers evaluation, data collection and research projects that provide information needed for policy development.

U.S. Department of Housing and Urban Development: HUD's mission is to increase homeownership, support community development and increase access to affordable housing free from discrimination. To fulfill this mission, HUD embraces high standards of ethics, management and accountability and forges new partnerships-particularly with faith-based and community organizations-that leverage resources and improve its ability to be effective on the community level.

U.S. Department of Labor: DOL is a widely diverse agency whose activities include law-enforcement programs such as the occupational safety and health and mine safety and health programs, wage standards and child labor programs and programs to protect the pensions of American workers. Other programs include workers compensation and unemployment compensation, job training initiatives and a variety of informational and statistical activities.
