On-Time Quality Improvement for Long-Term Care: Training, Diffusion and Implementation Project Plan

IFAS is partnering with the Institute for Clinical Outcomes Research on the On-Time Quality Improvement for Long-Term Care program, funded by the Agency for Healthcare Research and Quality. The program is designed to improve day-to-day practice in nursing homes through the improvement and redesign of workflow.

The program aims to:

  • Design, implement and evaluate a process using automated standardized forms and an information technology tool to implement best practices.
  • Track the impact of this process on workflow in key operational processes, including documentation, quality reporting and care planning.
  • Track the impact of this process on the rate of pressure ulcers acquired by high-risk residents.

IFAS collaborated with the Delmarva Foundation for Medical Care, the quality improvement organization for Maryland and the District of Columbia, to develop an organizational readiness assessment tool. This instrument is designed to help nursing homes identify and focus on the concrete issues they must address before undertaking facility-wide quality improvement initiatives. The tool was successfully piloted as part of the On-Time project but continues to undergo further development.
